Individualized Treatment Plans

Astoria Skilled Nursing and Rehabilitation provide a spacious rehabilitation gym for physical, occupational and speech therapy. Our enthusiastic therapy team will complete assessments and provide therapy 7 days a week. Together we will determine your individual goals to promote a strong recovery and safe return home. Specialized programs include orthopedic rehabilitation and cardiac rehabilitation, as well as post stroke and post-surgical recovery.

Physical Therapy treatment plans often include lower extremity dysfunction, training in area of bed mobility, transfers, gait, balance, strength, and coordination. Available physical agent modalities include ultrasound, TENS (transcutaneous electrical nerve stimulation), HealthLight™ and ESTIM.

Occupational Therapy addresses important elements in the treatment of individuals experiencing functional limitations in self-care skills, referred to as ADLs (activities of daily living). Treatment plans often include: upper body strengthening, dressing, bathing, grooming, home management, fine and gross motor control development as well as compensatory strategies.

Speech-Language Pathology guests are evaluated and treated for hearing, language, cognition, communication and swallowing disorders. Our focus is on improving speech-language expression, comprehension, and oral motor skills to enhance accurate speech production and strategies for safe eating. Treatment plans often include: reception/expressive language therapy, aural rehabilitation, dysphagia, cognitive ability and VitalStim®.
